Hot Flushes Are the Headline. Your Gut Is the Real Story


Hot flushes, night sweats, mood swings, brain fog, these are the symptoms that tend to dominate conversations about menopause, and for good reason. But a growing body of research suggests the gut may be playing a bigger role in this transition than most people realise.

Why Menopause May Affect Your Gut

Perimenopause typically begins in a woman's early-to-mid 40s as oestrogen and progesterone levels start to decline. Oestrogen isn't only a reproductive hormone, it appears to influence immune response, mood, and the composition of the gut microbiome.

Research by Baker et al. (2017) found that higher oestrogen levels are associated with greater gut microbiome diversity [1]. As oestrogen falls during perimenopause, the balance of gut bacteria may shift, a state researchers call dysbiosis, which has been linked to bloating, constipation, diarrhoea, and IBS-like symptoms that can appear to come from nowhere in your 40s.

The Estrobolome: A Possible Mechanism

One area of active research is the "estrobolome", a group of gut bacteria involved in metabolising oestrogen. When these bacteria are in good shape, they help regulate how much oestrogen circulates in the body. When the gut is disrupted, this process can become less reliable.

Kwa et al. (2016) looked at how microbiome changes may affect oestrogen metabolism [2]. Some researchers suggest this could be one factor contributing to perimenopause symptoms, though this is an area where the science is still catching up with clinical observation.

What Your Gut May Be Signalling

Stool patterns, frequency, consistency, transit time, can shift in response to hormonal change, stress, and diet. Sometimes these shifts are noticeable before other symptoms become obvious. They won’t tell you what’s wrong, but tracking them over time can give you something more concrete to discuss with a doctor.

Five Ways to Support Your Gut During Menopause

None of these replace medical advice, but each has a reasonable research base behind it.

1. Eat for diversity

Aiming for around 30 different plant foods per week, vegetables, fruits, legumes, wholegrains, nuts and seeds, is associated with greater microbiome diversity in observational studies. It’s less about any single superfood and more about variety overall.

2. Include fermented foods

Kefir, natural yoghurt, kimchi, sauerkraut, kombucha and miso introduce live bacteria and may support microbiome diversity. A 2021 Stanford study (Wastyk et al.) found that a high-fermented-food diet increased microbiome diversity and reduced certain inflammatory markers in healthy adults [5].

3. Watch the ultra-processed foods

Emulsifiers and certain additives found in ultra-processed foods have been associated with microbiome disruption (Chassaing et al., 2015 [6]). The research is still developing, but cutting back is broadly consistent with dietary guidance anyway.

4. Take stress seriously

Cortisol appears to influence gut permeability and microbiome composition (Karl et al., 2018). Physical activity, decent sleep, and anything that genuinely helps you wind down aren’t just nice-to-haves, they may have a direct effect on gut function too.

5. Protect your sleep

The gut microbiome appears to follow a circadian rhythm, and disrupted sleep may affect how it functions (Thaiss et al., 2014 [7]). During menopause, when night sweats and anxiety can already chip away at sleep quality, it’s worth treating sleep as a gut health issue as much as anything else.
